PAYMENT and INSURANCE
Fees vary depending on the type of service performed, the length of the visit, and the need for special tests. Fees for service are consistent with those accepted in the community and established by Medicare. We will be glad to discuss these with you and provide an estimate of the expected cost of any proposed procedure.
The practice is a participating practice and Dr. Drake is a participating physician with Medicare and Medicaid. We are also members of most local PPOs and HMO’s and will be happy to file claims for you. We accept assignment on all claims. Any co-payment or deductible is due at the time service is rendered.
Completion of special insurance or legal forms may incur a nominal fee.
